However, many older patients do not have the necessary bone density in their jaws to support a full set of implants. Until recently, this left them with only one option, a full set of unwieldy dentures. This option is not one that most people wish to live with. Recognizing this need, a system of implant based replacement was developed called All-On-4®. Patients can now enjoy the ease of living with a full set of teeth without the embarrassment and inconveniences associated with dentures.

How Does the All-on-4® Procedure Work?

While there may be minor variations on the All-On-4® treatment technique from one prosthodontist office to another, a typical All-On-4® procedure will consist of three distinct phases:

  1. Existing decaying, broken, or otherwise compromised teeth are removed. The diseased and damaged tissue is also removed.

  2. Two titanium implants are embedded in the front of the jaw where there is typically a greater concentration of atrophy-resistant bone tissue. Two additional implants are inserted in the back of the jaw at an angle (typically between 30 and 45 degrees).

  3. The prosthodontist will attach your new prosthetic dental bridge to the implants and adjust for comfort and fit.

The entire procedure can be completed in less than a day, although the healing process may take 3 to 6 months. Once the healing process is complete, a final prosthetic dental bridge is attached thus completing the All-On-4® process.

The Upside

  • They are more secure than traditional dentures and are removable.
  • There is no need to remove natural teeth that are still healthy.
  • Snap-on dentures fit over any remaining teeth and attach directly to them with a snapping action, creating a comfortable but secure and snug fit.
  • If some natural teeth are missing, implants can be embedded in the gums and act as supports for the snap-ons.
  • They are less likely to rub against the gums and create irritation since the devices attach to the teeth or implants and not the gums.
  • Are less expensive than partial or complete denture implants.

The Downside

  • It is possible some existing teeth may require filing into a shape that will ensure a better anchor for the overdentures.
  • Over time the fit may not be as snug due to changes in the mouth or gum line and could require a new set.
  • Can take up to 6-12 months for the final set.
  • They are bulky.
  • Snap-in dentures need constant replacement. They typically last two to 10 years.

The Upside

  • Dental implants offer permanence. Once the procedure for placing the implants is complete, your tooth is permanently fixed to the jaw and does not easily get loose.
  • A good prosthodontist can make these look like natural teeth.
  • Implants are much more comfortable because they look and feel like real teeth with no telltale “clicking” noises or speaking and chewing difficulties that occur with dentures.
  • You care for your implants in the same way you care for your real teeth — with regular dental visits and good oral hygiene.

The Downside

  • They are much more expensive than other newer options that are available. Because of this reason, it is often considered a last resort.
  • Dental implants aren’t a quick fix like the All-On-Four. These are often a 1 to 2-year process to completion.
  • Not everyone is qualified for dental implants.

Most of these tooth replacement solutions don’t feel natural nor do they last for an extended period of time due to a variety of reasons. They are, however, quite affordable and require no dental surgery.

Unfortunately, for patients who want a tooth replacement solution that looks, acts, and feels natural, traditional removable dentures are far from ideal. Removable dentures aren’t structurally anchored into a patient’s mouth like dental implants are. As a result, they often shift or can even accidentally fall out of a person’s mouth.

Once upon a time, the gold standard for a natural, long-lasting, and good-looking tooth replacement treatment relied on the use of conventional dental implants. With a conventional dental implant procedure, each tooth must be replaced with its own dental implant and crown prosthesis. As you can imagine, in cases of full mouth reconstruction or other extensive dental or maxillofacial restorations, replacing each individual tooth with an implant can become quite cumbersome and quite costly.

Replacing 32 teeth with individual implants not only requires enormous time and effort but it also places a tremendous physical strain on a patient’s health. Furthermore, many conventional implant procedures also necessitate a prior bone grafting procedure adding to the complexity, cost, and recovery time patients must endure.

On the other hand, patients who decide that they just cannot afford or don’t want extensive, invasive oral surgery via conventional implants are stuck with traditional dentures.

While considerably more affordable than more permanent solutions, dentures also don’t last very long. Regular daily wear and tear often necessitate denture replacements every few years.

Patients with bruxism, or teeth grinding, or who have particularly strong bites can find themselves replacing their dentures even more often. In comparison, dental implants can last decades or even a lifetime when cared for and maintained properly.

Rather than face 32 individual implants into the jaws for a complete set of two arches, patients only need eight individual implants. The sheer difference in the scope of the intervention is for many the sole reason they opt for All-On-4 dental implants over conventional dental implants.

The drastic reduction in invasiveness and incisions into the sot and hard tissues of the mouth and jaw mean remarkably less potential for complications, such as a severed nerve or postoperative infection.
